A 482 visa does not by itself give Medicare. Medicare eligibility is governed by the Health Insurance Act 1973 and is generally restricted to people permanently residing in Australia who are Australian citizens, permanent visa holders, New Zealand citizens or, in certain circumstances, applicants for permanent residence visas (Home Affairs, Adequate health insurance for visa holders, updated 13 April 2026). A 482 holder from one of the 11 countries with a reciprocal health care agreement may be eligible for medically necessary care under Medicare. Everyone else pays their own health costs, which is why every 482 carries condition 8501, the duty to maintain adequate health insurance (Migration Regulations 1994, clause 482.612).

Can a 482 holder get Medicare?
A 482 holder gets Medicare only if a reciprocal health care agreement covers them or, in certain circumstances, after applying for permanent residence; the 482 itself carries no Medicare entitlement. Home Affairs states that visitors from a country without an agreement who need medical care have to pay the full cost (Home Affairs, Adequate health insurance for visa holders).
Through a reciprocal health care agreement
Services Australia describes the agreements as covering “the cost of medically necessary care” when visitors from the 11 countries come to Australia, and says a visitor from each of them “may be eligible” for care under Medicare while here (Services Australia, When Reciprocal Health Care Agreements apply and you visit Australia). For Italy and Malta the period is 6 months; for Malta, cover starts on the day of arrival and ends after 6 months unless the visa ends earlier. Enrolment happens with Services Australia after arrival.
After applying for permanent residence
Home Affairs lists applicants for permanent residence visas, in certain circumstances, among the people Medicare can cover. Services Australia sets those circumstances and decides each case, so a 482 holder who lodges a 186 or another permanent application checks with Services Australia before relying on Medicare.
Which countries have a reciprocal agreement?
Australia has reciprocal health care agreements with 11 countries: Belgium, Finland, Ireland, Italy, Malta, the Netherlands, New Zealand, Norway, Slovenia, Sweden and the United Kingdom (Services Australia). Eligibility for these arrangements is assessed case by case, and a person cannot enrol in Medicare until they have arrived in Australia (privatehealth.gov.au, Working visa applicants).
What does reciprocal cover include?
Reciprocal cover is for medically necessary care, and its scope differs by agreement, so the Services Australia entry for the country sets out what is covered. It is not a substitute for the health insurance condition 8501 requires unless Home Affairs grants an exemption, as the next section explains.
Do you still need private health insurance?
Yes, in almost every case. Condition 8501 applies to every 482 holder whether or not they can enrol in Medicare. The one exception: a holder covered by an agreement and enrolled in Medicare can apply to Home Affairs for an exemption from the insurance requirement (privatehealth.gov.au, Working visa applicants). An applicant outside Australia still needs complying insurance first, because Medicare enrolment happens only after arrival. The guide to health insurance for 482 visa holders sets out the minimum benefits Home Affairs lists.
Can a 482 holder claim a Medicare levy exemption?
A 482 holder who is a resident for tax purposes and is not entitled to Medicare benefits can claim the Medicare levy exemption for that period, which needs a Medicare Entitlement Statement from Services Australia; Services Australia can take up to 8 weeks to process an application for one (ATO, Not entitled to Medicare benefits, updated 22 June 2026). The levy itself, 2% of taxable income, and how it is withheld from pay are explained in the guide to tax on a 482 visa.
- Foreign residents: a 482 holder who is a foreign resident for tax purposes does not pay the Medicare levy and can claim an exemption for the days of the income year they were a foreign resident (ATO, Foreign and temporary residents).
- Family members: whether a spouse’s or dependant’s Medicare entitlement affects the exemption is set out on the ATO page; check it before lodging the tax return.
Supplementary: 482 conditions, and Medicare once you hold PR
Condition 8501 stays on the 482 for as long as it is held. Permanent visa holders are among the people Medicare covers. The subclass 482 visa guide covers the visa’s other conditions, and the guide to 482 visa to PR sets out the routes to permanent residence.
An Australian migration lawyer makes the next step more likely to succeed here because Medicare access for an applicant for permanent residence depends on the circumstances Services Australia applies to a valid, pending application, so the validity and timing of the 186 or other permanent application decide whether a 482 holder can rely on public cover.
Questions people ask
Am I eligible for Medicare on a 482 visa?
Only in limited cases: if you come from one of the 11 countries with a reciprocal health care agreement and Services Australia accepts you under it, or in certain circumstances once you have applied for a permanent visa. Services Australia decides eligibility.
Do I need health insurance on a 482 visa?
Yes. Every 482 carries condition 8501, which requires adequate health insurance for the whole stay in Australia, whether or not you can enrol in Medicare.
Do 482 visa holders pay the Medicare levy?
A holder who is a resident for tax purposes pays the 2% levy unless an exemption applies; a holder not entitled to Medicare claims the exemption with a Medicare Entitlement Statement. A foreign resident for tax purposes does not pay the levy.
Can a 482 holder get Medicare after applying for PR?
Possibly. Home Affairs says Medicare eligibility extends, in certain circumstances, to applicants for permanent residence visas. Services Australia decides whether a particular applicant can enrol.
What is a Medicare Entitlement Statement?
A statement from Services Australia used to claim the Medicare levy exemption for a period when you were not entitled to Medicare benefits. Services Australia can take up to 8 weeks to process an application for one.
